pEast River Medical Imaging PCppNotice of Data Security Incidentpp ppEast River Medical Imaging ERMI takes the privacy and security of patient information very seriously On November 22 2023 we began sending notification letters to patients and employees of a security incident involving some of their informationppOn September 20 2023 ERMI identified suspicious activity within our IT network We immediately initiated our incident response process began an investigation with the assistance of a cybersecurity firm and notified law enforcement The investigation determined that an unauthorized party accessed our network and between August 31 2023 and September 20 2023 accessed andor copied some documents on the systemppThe documents involved in this incident contained information that varied from person to person Depending on the person information impacted may have included name contact information insurance information exam andor procedure information referring physician andor imaging results andor Social Security number For employees the information reflected names contact information financial account information Social Security number andor drivers license numberppWe have and will continue to take steps to enhance the security of our computer systems and the data we maintain To help prevent something like this from happening again we have enhanced our network monitoring capabilities and will continue to assess and supplement our security controls going forwardppWe are offering individuals whose Social Security number or drivers license number was impacted complimentary credit monitoring services Enrollment information is included in notification letters sent to those individuals We recommend as a best practice that patients review statements they receive related to their healthcare If they identify charges for services not received they should contact the healthcare entity or health insurer immediatelyppWe remain committed to protecting the confidentiality and security of information we maintain and apologize for the concern this may cause If patients have questions they should call 18006304065 available Monday to Friday 900 AM to 1100 PM ET and Saturday and Sunday 1100 AM to 800 PM ET excluding major US holidays Please be prepared to provide engagement number B108138p
