2024 Consulting Radiologists LTD CRL is providing notice of an event that may affect the privacy of certain individuals information CRL takes this incident very seriously and is providing information about the incident our response to it and resources available to individuals to help protect their information should they feel it appropriate to do soppWhat Happened On February 12 2024 CRL detected suspicious activity in its network environment Upon discovery of this incident CRL promptly took steps to secure its network and engaged a specialized cybersecurity firm to investigate the nature and scope of the incident As a result of the investigation CRL learned that an unauthorized actor accessed certain files and data stored within its network Upon learning this CRL began a timeconsuming and detailed reconstruction and review of the data stored on the server at the time of this incident to understand whose information was affected On April 17 2024 CRL identified persons whose sensitive data was included within the impacted data At this time CRL has no evidence that any of the information has been misused by a third partyppWhat Information Was Involved While CRL has no evidence that any information has been misused CRL is notifying impacted individuals and providing information and resources to help protect individuals personal information The following types of information may have been impacted name date of birth address health insurance information and medical information Small subsets of patients had their Social Security number or drivers license number impacted and another small subset included face sheets and imaging reports The type of information at issue varies for each personppWhat We Are Doing CRL takes this event and the security of personal information in its care very seriously Upon learning of this event CRL moved quickly to investigate and respond to the event and notify potentially affected individuals As part of its ongoing commitment to the security of information CRL is reviewing and enhancing its existing policies and procedures related to data privacy to reduce the likelihood of a similar future event CRL is notifying impacted individuals for whom CRL has a valid mailing address via US mail and for those whose Social Security numbers were impacted offering them credit monitoring and identity protection services CRL is also notifying applicable regulatorsppHow Will Individuals Know If They Are Affected By This Incident CRL is mailing a notice letter to individuals whose information was determined to be in the affected files for whom a valid mailing address is available If an individual does not receive a letter but would like to know if they are affected they may call CRLs dedicated assistance line detailed fraud by reviewing your account statements explanation of benefits forms and monitoring your free credit reports for suspicious activity and to detect errors Under US law individuals are entitled to
